What is a birth plan?
A birth plan is a way to communicate preferences before labor. It can help you and your care team discuss what matters to you, what is available and what might need to change. It is not a guarantee of how labor or delivery will unfold.
Start with communication
Write down how you would like options explained and who you want involved in conversations. Add comfort preferences and questions about pain relief, feeding and newborn care. Leaving a section blank is fine. This template deliberately starts without selected medical interventions or automatic treatment preferences.
Review it together
Bring the draft to a prenatal visit. Ask which choices are feasible at your birth location and how preferences can be adapted if circumstances change. Keep the final document short enough to scan quickly. The printable sheet contains only the information you enter and a reminder that preferences may change; it does not create medical orders. Text stays in the open page and is not saved automatically. Download a text copy or print before leaving.
